X-15 Flight 191 took place on November 15, 1967. It was piloted by NASA pilot Michael J. Adams. The flight lasted a total of 10 minutes and 35 seconds. On November 15th at 10:30 in the morning, The X-15 dropped from the NB-52B mothership at 45,000 feet above Delmar Dry Lake. An electrical disturbance hindered Adams' control during the beginning of the flight. Adams chose to continue the flight. As the aircraft ascended, Adams began a planned maneuver that would allow a camera on the aircraft to scan the horizon. However, when the aircraft reached its maximum altitude, it was 15 degrees too far to the left. The aircraft was able to correct itself but while descending the aircraft entered a Mach 5 spin. The engineers had no way to monitor the path of the X-15. Adams was able to contact the control room when he entered the spin. There was no spin recovery technique for the X-15 and the engineers didn't know that the spin was at Mach 5. Adams was able to hold the flight controls for the aircraft and he was able to recover from the spin. As soon as Adams recovered from the spin, the aircraft entered a Mach 4 dive at 36,000 m from the ground. As the aircraft neared 20,000 m it was diving at Mach 3.93 and was experiencing more than 15 gs vertically. Northeast of the town of Johannesburg, the aircraft was torn apart and Adams died in the crash. The crash of the X-15 and the death of Adams caused a telemetered heading indicator to be put in the NASA control room. The crash was most likely caused because of the electrical surge or because Adams may have experienced vertigo. A memorial was made of Adams at the crash site by Boy Scout John Bodylski and Adams' name appears on the Space Mirror Memorial.
